How to Obtain your Medical Marijuana ID Card

STEP 1: Create an Account On–Line

For adults 18 years of age or older or a minor patient with a caregiver omma.ok.gov

  • Click on Apply; Click on Register
  • Enter a valid email address and create a password
  • Check for “Confirm Your Account” email (you may need to check your trash or spam folder)
  • Confirm your account; Begin your application

STEP 2: Complete Application

  • Name and proof of identity; Address and Proof of Residency; DOB; Phone; Email Address
  • Caregiver info if applicable
  • Passport–like photo
  • Physician/Podiatrist info and Recommendation Form dated within 30 days of application submission
  • Sign by typing in full legal name and date

STEP 3: Pay the Fee and Submit Application

  • Standard 2–year application fee $100
  • Short term 60–day application fee $100
  • Medicaid (Sooner Care), Medicare, and 100 % disabled veterans, 2–year application fee $20

STEP 4: Obtain ID Card

Card will be mailed within 14 business days

Caregiver Instructions

STEP 1: Complete Caregiver Application On–Line

Minor patients may have up to two parents or legal guardians omma.ok.gov

  • Name and proof of identity; Address and Proof of Residency; DOB; Phone; Email Address
  • Then name and the 24 character medical marijuana license number for the patient who has designated you as their caregiver
  • Passport–like photo
  • Sign by typing in full legal name and date

STEP 2: Obtain ID Card

Card will be mailed within 14 business days

REQUIREMENTS FOR OBTAINING A MEDICAL MARIJUANA LICENSE ID CARD

  • An Oklahoma licensed and practicing physician or podiatrist may recommend medical cannabis for any condition they believe will be improved through the administration of medical cannabis
  • A medical record and a bona fide relationship must exist with the patient
  • The physician or podiatrist must recommend medical cannabis according to the accepted standards a reasonable and prudent physician or podiatrist would follow for recommending or approving any medication
  • If applicable, the physician or podiatrist may certify that the patient is homebound or does not have the capability to self–administer or purchase medical cannabis due to a developmental disability or a physical or cognitive impairment; and the physician or podiatrist believes the patient would benefit from having a caregiver with a caregiver’s license designated to manage the patient’s medical cannabis on the patient’s behalf.
  • The physician or podiatrist must verify the patient’s identity as provided in the Oklahoma Medical Marijuana Authority
